Our practice was at first flntiiatiii(?, as was llio case, indeed, with most, other phy sicians at the onset of their treatment of this dismast". I'liunrliil ilimitlalion and a free ue of caliiniel were principally depended on at llio c iin'iieiiceiiieiilj but, 11 ml ing Hint inticli sliiniiliilion (as it appear lo me must always bo ihe case) increased the general oppression, and the congestions of the viscera, rather than lessened them, it waa abandoned in liivor of the lirandu 1'tiimr Huffs, in (iinjitlctinii with raloniel, luuslard pluslera, and warm, sliirutly slim-ulaling drinks, in the early etago of thedisease. With this alteration, niirsttcccsa so muc'i imnrnveil. that when tin) patient was in troiluci'd into the Hospital immediately after the attack, and I lie irritability ot tin tinnncli was not loo great, nnd the lenden cy lo coll.ipsu not (oo highly developed, ve succeeded, inostlv. in restorinff them. When llio reverso was the cae, death (jo-nerally proved inevitable. Hoietics undo no part of the treatment, at least, not to any extent. It was not until after the disease had disappeared from the city, that reflection led too lo contrast pome of iti more prominent feature with those peculiar lo the malignant levers i.i the southern Slates, n very ably described hy l)r Cartvrilil, in the :):lril, Hlili, mid 3"iilt numbers uf llio American .Medical U'' order. The Doctor's close nb-airvatim, and his great industry in dissecnons in tins lever, led htm to the discovery, to n his own lni?ua)je,of a "diseased siateof lite gam;-lions, the (janclioniry nerves, and the in-fl iininali'in of their inventing inemhraues.'' If tliis then, was III" leading pilholojiiciil condition in these malignant fevers, in h w iiueli grratrr dtgret must it bo present in Cholera, where the (raiiolioiiary nerves, or those of organic life appear, as it were, in a completely futralgstJ condition, which every nun will agree is more terrible than inflammation! it by no iiienna needs the nid of dissec tion, to prove thai the great, and perhap" the primary ililUciiliy in this disease, is aealed HI this i-yslPin of nerves. This, every observer would readily infer, Irnm the fact that the luuctions ol 'organic life, are the fir, and al -t the only ones lo aufl'erin an altick of this dreadful malady. Whether this portion of llio nervous system is in an inlUmod, congested, or paralysed condition, (ihe latter of which op-lieara the most plausible.) is a matter ol' little moment, compared with the ascertainment of Ihe mn.l judicious and successful plan of treatment. Dr. 0., with a boldness and disertminn- lion worlhy of all praise, marked out lor himself, what was considered nl thai tune, and even now, a novel mode, of procedure in the disease a Inch he has so ulily 4c-ucrihed. It consisted in Ihe use of auti ' nouial emetics ill the first stage, or thai of 'ataxic re aciioni" and his success con-firmed his views. These lie gave, if possi.j ble, in that stage of crippled excitement, "tfiieu the blond is uiuually iluiernniu'il; the heat ummtrilly dill'iiscd, sensation impaired, and secretion suspended," which immediately precedes collapse, or what is much more rare, re-aeliou. From lite success which followed Dr. C'trtwrighl's practico, in this particular, I felt convinced that we had not given emetics a sullicient trial in 111) tieiitmeut of Cholera, and determined. Ilierelore, fully to test their ellicacy, in cn-o the disease should aflorwatds reviBit us. In the mean-tine, I had noticed, during its prevalence hern in tho fall of H:1J, lint the Thump-oniaas frequently arrested fur a time, its yomit'ng and rapid progroH-i, by resorting oarly to LoMin. tmelirt. This temporary arrestton, however, proved of Utile avail, eilbor to themselves or their pitients, as, having no knowledge of pathology, and no deflnite object in view, tiny withheld, at the most important ciiais, when tho vomiting was allayed, and the ayslam reacting, those remedies usually o nuloyod for the restoration of suppressed secretion. l.o beha. like all oilier emetic, mil. 'si given very early in the disease, is not remarkable for its pervading inlloence over tho secre toryfouctiona. It will over toui" ihe nion appalling features of inotbid action ill One li ra, bin aoiK-ars loexeil lilll deobstrueui effect on the secernent easels. This ap oeanid lo be llm case in iheir practioej con ,cnuently their patiutils, after lyn'2 throi or four days (as I have freoionily seen,) in a stale of broken or partinl excitement, willi every fui.clion of sec re I inn c pletely locked up; sunk into collape, and died. The disease, contrary t i i ur anticipation, having unfortunately again appeared aai'Uig-i us last soring and sumiunr, an op-oorliMity was alPirded me for te-tmg the utility nf emetics in caso4 in which the excessive irnt'inility of tho sto.oach, nnd the rapid tendency to collapsn, nil'. riled lit lie reli-uice en Ihe curative (lowers of calomel. Now, in the stage of premonition, where there is Ittlle el-e than the diarrlioji and approaching prostration present, the cure can nltnost universally be elF-cted hy the nso of dry external unrmlli, hot and inoderiitely stiuiiilaiing drinks, together with calomel and opium, so given, as to allay irritability, check tlm evacuation-, equalize the circulation, and arouse the so. ereti ois generally, especially that of ihe liver. When this is neglected, and the disease passes on to vomiting, spasms, great thirst, iitseu-ihil.ly, and renin rkable feebleness ol nnl-e, Hivro is nothing in my opinion, in the whole range of therapeutic ngenis, equal in lli. ncy to the operation of a pro-ner emetic. 1 do not look upon them n-being able, in gei-ral, lo cure the disease) llieir province is lo arrest it, mid so to thwart the uinrlvd actions of the system, arouse susceptibility, and dispeise internal eniigest i.m, as to allow cal nuel to lie upon Hie stomach, and give it tiino and opportunity to unlock the secretions. W here Ihe vomiting has become frequent and violent, nnd the system is sinking ripidly iuio collapse, this latter cll'.cl cannot be obtained hy the calomel, as it can rarely be retained in the stomach to exceed a lew momenta, nt lenst, not long cnoii;'li to ex ert any specilic iiilloeuce on the torpid system. Kven where there is liltle or no vo. oiling mid spasm, hut much torpor and tendency in pros! ration pre-eul, an emetic is frequently of great advantage, in nroiis-iirr a higher degree of susceptibility to I lie inlliiei.ee nf calomel, u ml in equalizing the circulation. l lio emclic which I have found most successful, and which I ino-t value, is a combination of coiuinoii salt, (niurialo ol soda,) and ground iiiu-taril, mixed in loautily ol warm water, suiheient lo tit solve and dilute them, ns circiiinslr.iices oi-iy require. Where the torpor and pros- tr iiion is nut al inning, the s ill water alone tnav or.eliieo the desired etr'ci; but wheie the reverse is the case, the mustard should be added, and increased in u amity, ac cording to tho degree of torpor and insen- sihihiy present. Very little regard need lie paiil to quantity; from one t i two table spoonsful of each must hi; administered, when collapse is fast approaching, nnd repeated unlil the vomiting and excruciating disl ress in llio epigastrium entirely subside, mil evidence of reaction nppears. Coui-Inneilly inki-n, they stimulate the coats nl i ho stomach much more, and operate in less lime, and with more power. They appear to have a stiiniilnnl ellect on the sioiunch, dill'orent, and moro congenial, than that Iroin any other stimulating sub at lice, which I have ever seen administered in Cholera. I value the Lobelia emetic very highly! but it is railn'r more prostrating, and cannot be repented ns often as might be ilesireit. 1 lie nuliiiiouial emetic, though sanctioned by Dr. (' irtwriglit, in the iiiiliguanl fevers of the south, and advocated by a few in Cholera, appears even less worthy of contideuce thnn the l.ohclis. 1 look upon it as more relaxing and exhausting; while the sail nod iiiii-lnid have Ihelwo fold property of h'ing looic Hid stiiuolinl, or excitmil. 1 have frequently seen these Inner act nlnio-t like a charm, in subduing all Ihe untoward symp toms ol a malignant case, and have there-lore a confidence in them, bordering a little, no doubt, on enthusiasm, yet I must insist that it is well founded. lion of Ihe internal viscera, be able to (lis. perse their congestions and equalize the general circulation. Accordingly, 1 prepared a mixture of the salt and mustard, rf each a largo Inble Riioonful, in half a pint of warm water, and administered it during the deadly nausea ami oppression, winch iininedia'ely precede a paroxvsin of full vomiting. Thi- ooera ed freely, and with some apparent l-enelii. A'tor repenting it live or six limes, the intervals between tho eflorts at vomilinir grew much longer and more quiet. and tho system gave evidence of a disposi tion to rally its energies. When it had lieen taken eight or ten tunes, the interval became so period that the pa';"ei enjoyed a few ininines quiet sleep, nl'ler c ,nrnx- ys o, which continued until a -used hy another approach of the distressing nausea; when nnoilier portion of tho emetic was i nun 'iliniely given, nnd with the usual good ellect. This preparation was continued unlil it bad been given. upwards of twenty times; a'tor which it ceased to produce any euiniic effect, the two last portions hiving been reiaitool cei the stomach. This stale was immediately followed lV an amelioration id' all the symptoms; the spasms entirely disappeared; Hie unusea and p-nntol op. pression subsided; the pulse, though nearly extinct, soon became toller, so'ler, and less frequent; sensibility returned, logei'ier with warmth of surface, and an equalization of circulation, followed hy an agreal.le mnl general quiet. 1 then gave nn anodyne, I find that I have extended my remarks lo a much greater length than was anticipated when I com neuced; and though I hnvo given but n mere outliuo of the sub ject, and have avoided the iiiinuiiffi nnd leciiuie iiiiies of a lornial e-sny, I shall tool satis ied without further apology, if tin-hints thrown out shall in any way pr.ve ii str nnoiital in calling the attention of the profissiou more fully to the importance ol his treatment, C.nriniitiii. In order to show the reason for my coti-liilence in llio repealed and erseveriiig u-e of this kind ol emetics, in certain coudi-dons of ibis complaint, I will give the out lines of a case ir two, wherein they very much exceeded my anticipations of Iheir virtues and Illness in the disease, which may serve to show that my llc-ry is limn deij to some exieot in experience, whether it he citrrect or not. The first case which camo under tny treatment, that could not be arrested hy Hie use of anodynes, calomel, and external heal and irritants, occurred on the 1th of .Inly, H'l:t. The pat t, .Mr 0. II. ,McN.,was ML years of age, of sangiiiuo temperament, inn keeper by occupation, and possessed a good couslitulion, except tint ho hid suf tepol somewhat I'ro.o autumnal fevers. Kour or live hours heiiire I saw him, in addition to his dinrrl.u) i, he wasatlacked with violent v. muting, spasms, exireiue thirst, cold clammy sweats, and great oppression in the epigastric region. I found his puls frequent, small, uud very feeble; his conn every three hours, until live were given" which Ireely moved the bowels, and general restoration nf secretion soon followed. Thus the result which had been so anxiously desired, but so lift 1 0 anticipated, was soon cfl'-cied, and the consecutive fever being slight, the patient shortly recovered. The next despernto enso in which emetics of salt and mustard combined, evinced extraordinary powers in arresting llio disease, occurred on llio Hill of July, only 'our days subsequent to the one ju-t mentioned. The patient was a female, aged about .).); had sull'ired nil attack about ten days previous, from which she had recovered; hut, from imprudence in diet, hid b'-en taken Willi a relnp-e, which apparently prostrated her beyond nil hope of recovery in a ver; short time. Two physicians were iiinnediiitety Miuioioued t her aid; but as every symptom evinced such n high degree ol lualigoiiy, noil me case, withal, npnonred so utterly h .peless, lint very little waa attempted, before they gave her up. Her trieiids waiiou tour or live hours to seO her expire, but accidentally hearing of my success in the heloto inen-lioned case, immediately called mo to visit her. I found her laboring under every symptom of tho most aggravating mid in-lenso kind, nnd to nil iipp-arance, in the very last moments ot existence. I wns unable lo disc ver any pnlsntinn at llio wrisl; Ihe vomiiiug and rice water evacuations had considerably sub-ulcd, in tho general insensibility and relaxation; tho spasms were becoming lo s severe; the mouth, tongue, and extremities were verv cold; the wb do surface drenched in cold and clammy in n-lu.e, and tbq siht and hearing nearly lost. I did not hesitate for a moment, in a rree. ing with the other physicians, h md see. i the case, that it was usecsS ,, disturii her, and so expressed myself to her friends; but linding iny-elf uuilile to leave, mini something should he d mo, without giving .dl'ence in the family, 1 concluded to Ii,. 'iliniely, by mixing two table soo uisful ol each, in lull' a pint of wnrm water, and ailmiuisicrcd it to her ns soon as she could be made lo swallow, her deg lutition being very dillicult. This, to my great surprise, proluced a powerful action of the stomach in about Ijllcon minutes. Alter the vomiting was uver, I gav" her another portion, the opern-lion of wlreh she described lo me as making her f.-el ns if her "stomach w is burning up, and streams of tiro were flowing "ill. in every directum throughout her sy.. torn." Sullice it lo say, that in a lew hours after, and live or six operations of this kind, we succeed. d til quieiiug iln-system, in raising the pulsp, warming the surface to some oxt-mt, mid in arousing the sensibility and equalizing the circulation, rinnn.'ls dipped in hot spirits of turpentine were applied to the spine, ami mu-l iril plasters to tin) epigastriu o, at tho same time. u anodyne wns llco given, and scruple doses of calomel ordered to ho repealed every three hours, until tlie evacuations became C'.n-isleut, and evidence of a general restoration of secretion aope-tred. slight consecutive fever toll owed, which evinced llieu-ii il disposition to fusion itself upon some ouo or two of lit) more vital organs, more particularly that of Ihe brain.
