Obtain the Submission Form
There are three ways you can make your submission to the Ohio Memory Project:
- Download a copy of the Complete Submission Packet (see below), print it out, fill out the requested information regarding your submission, and mail the form to:

Download a copy of the Complete Submission Packet (see below), enter the information regarding your submission directly into the Microsoft Word file, and E-mail the file as an attachment to ohiomemory@ohiohistory.org.
- Use the online form to make your submission. Note: You will need to first obtain a password, which you can get either from your Content Editor or by e-mailing us at ohiomemory@ohiohistory.org.

Note: The Microsoft Word files () were done in Microsoft Word 97 and can be opened by any later versions of Word or WordPerfect. Adobe acrobat files () are duplicates of the original printed copies and will format correctly regardless of the type of printer you have. You can download a free copy of Acrobat at the Adobe download site.
